COSTS & INSURANCE

Treatment is fee-for-service (which means that we require payment at the time of service), except for workman’s compensation patients (patient is responsible for any amount not covered by insurance). Insurance reimbursement forms are provided for services rendered, to help speed up your potential reimbursement.


INSURANCE

Many insurance plans DO reimburse for our services. PPOs reimburse up to 80%, but we are considered “out-of-network” providers. Non-local HMOs reimburse up to 100%. Health Savings Accounts (HSA) and Flexible Savings Accounts (FLEX) may be used. If you are concerned, please check with your insurance provider for their rules and regulations regarding reimbursement.



TRAVEL

$4.00/mile travel fee is added assessed from the center of Paradise Valley – for maintenance of our mobile unit. The fee may be waived if a physician is already in the area at the time of call for service.
Additional charges may apply for medications, procedures, supplies, and lab testing.


WELLNESS

Your wellness = Priceless


Our Housecall fees are less than many Urgent Care Clinics, and significantly less than your hospital ER. Since we do not have an office and large staff overhead, we are able to pass the savings on to you! Unlike concierge medicine, there is no membership required.
